Robert Downey Jr stars as a 17th century doctor who plays the buffoon in King Charles II's court and ends up in a sham marriage to one of the King's mistresses in return for a title and estate. He loses favour when he makes a pass at his 'wife', and ends up working with the mentally ill in the slums. A cured patient (Meg Ryan) becomes his lover, but twists of fate thrust them further into the mires of plague-ridden London.
